Injury Waiver:
 In the event I, my child, my caregiver, or my child's caregiver and their guests sustains an injury or illness due to attendance at The Duke Academy or participation in any session or activity hosted by The Duke Academy, I waive any claim I, personally or on behalf of my child, my caregiver, my child's caregiver and their guests, may have against The Duke Academy, its owners, affiliates, officers, directors, contractors, and/or employees and release and hold harmless such entity(ies) or person(s) from any loss, damage, liability, and expense that may be incurred as a result of the injury or illness, including death.

Media Release:
 The Duke Academy is hereby granted permission to take photographs, audio, video recordings, and live audio/video broadcasts of the student/client to use in any and all manner and media, now known or hereafter devised, throughout the world, for any and all purposes including, without limitation, in promotional print advertisements, websites, emails, instructional videos, and social media. Permission is also hereby granted for The Duke Academy to copyright such media in its name. Furthermore, I understand and agree that there is no compensation for any such use of media.

*Any suit, action or proceeding seeking to enforce any provision of, or based on any matter arising out of or in connection with, The Duke Academy services, terms, or policies or the transactions contemplated hereby may be brought in any federal or state court located in Jefferson Parish in the State of Louisiana.
